Personal Assessment / Interactive Questionnaire
This interactive questionnaire will enable you to gauge where you sit on the mental health wellbeing continuum. You will be asked a series of questions and you can respond to them by choosing words or pictures or moving an indicator as instructed. When you have completed the sections relevant to you, you will be able to view, email or print a summary and conclusion that will suggest many options available to you. Service providers, Information pages, website links, publications and guidance will be included in the conclusion.
Your information is completely anonymous and while we will keep statistics of all assessments completed there will be no way to identify the individuals who completed them.
Important things to note:
The answers you provide need to reflect your experience over the last two weeks and may be inaccurate if this is not considered.
There are many things that can contribute to feelings of depression. If you have had a baby recently or experienced a significant loss of some kind, your feelings of depression may be justified. It is only when these feelings continue for an extended period or are not related to these kinds of experiences that there is extra concern. If you have had experiences such as those mentioned above and are concerned about how long you have been feeling this way, we encourage you to seek a GP or Health Professional to talk to.
